Trial Membership / Visitor & Membership Waiver Form

Disclaimer:

We hereby apply to become either a trial member or full member of Two Kings Martial Arts PTY LTD fully realising that martial arts is a contact sport and involves fighting. We will indemnify from litigation or prosecution of the instructors, assistant instructors, or any agent of Two Kings Martial Arts PTY LTD (TKMA) for any situation which may arise from training and/ or championships whilst a member of TKMA . We will not use the skills taught to us by TKMA instructors unless our personal safety or that of my family are threatened and will only use as much force as is necessary to stop the attack. We will not behave in a manner so as to disgrace the name of Two Kings Martial Arts. This membership is subject to being revoked at any at any time without warning we fully understand that breaking any of the rules or regulations of Two Kings Martial, may result in our membership being cancelled without notice. 

Social Media:

You agree and understand that there may be times where you will be filmed and photographed.  Your pictures/ videos will only be used for our business accounts and social media. We do understand that some people may not want to be recorded due to court orders etc. and you need to let Shantal Van Moolenbroek know in writing that you/ your child cannot be filmed or included in group photos. 

Inclusiveness: 

We are an inclusive club, however if you have a disability or health issue that may hinder your training, we need to be made aware of this. We will notify you if we require a medical clearance or letter from your GP. You must provide documentation on request before attending class otherwise instructors can refuse you. If you require a carer the carer must be present during class, and we can request this if we think that you are mentally incapacitated or unable to make decisions for yourself or need any additional support. Thiis is for the safety of yourself, our students and the instructors.

Alcohol and drugs: 

You cannot train whilst under the influence of any illicit substance or Alcohol. If an instructor suspects that you are on any substance that may hinder your judgement both mentally and physically, they have the right to ask you to leave the club.

Cuts & Wounds

As we are a high contact gym you may not train with any open sores, Cuts, Ring Worm, Golden Staff, Warts. Please be considerate and if you have any of these infections, please take the right precautions seek medical treatment. Do not train whilst it is active and cover up properly when returning. If instructors notice that you have any of the above, they will refuse you the right to train.

General Hygiene: 

You must come to Class:

-Clean non smelling 

-Long Hair tied back

-Clean Uniform and correct Uniform that is required for your martial art.

-Correct Equipment that is properly maintained and not smelling 

-Nails must be kept short 

-No make up or strong-smelling perfumes 

-Deodorant must be worn

-Wipes are provided for feet and hand sanitizer, Bathrooms have deodorant and Sanitary Products in them. 

Behaviour: 

If at any time you behave in a dangerous, disrespectful way to any student, member or staff member you will be immediately banned from the club. Whilst at Two Kings Martial Arts you must remain respectful, follow all safety instructions and guidelines set out by your instructor.
